An inquest in 2003 concluded the 42-year-old's death was accidental.

His family are now calling for a fresh investigation into his death.

The inquest heard Mr Coles was above the drink-drive limit and walked into the horse's path but an eyewitness said he saw the horse being ridden “straight at Terry”.

The documentary will highlight how the witness’s statement was read out at the inquest but he was never called to give evidence, and how the inquest jury also never watched the CCTV.
